Prostate cancer is among the most common forms of cancer in men, but it is also one of the most treatable—especially when detected early. One of the particular challenges of this disease is that it often develops silently, without noticeable symptoms for a long time. As a result, many men tend to underestimate the issue or only discover it when it has already reached an advanced stage.

So, do all prostate cancers need to be treated surgically, or are there alternative options?

In fact, not all prostate cancers require immediate treatment. A significant proportion of cases are slow-growing and unlikely to progress to metastatic disease. These are classified as Grade 1 tumors, or what was previously referred to as Gleason score 6 (3+3)—a low-aggressiveness form.

The Gleason score is a crucial parameter in urologic oncology used to grade the aggressiveness of prostate cancer.
Patients with low-grade disease are often ideal candidates for active surveillance, a management strategy involving regular monitoring through MRI scans, repeat biopsies, and clinical evaluations over time. More than half of men in this category will never require surgery, radiation, or any aggressive treatment, which could otherwise be excessive given the indolent nature of their cancer.
